Health Insurance Contact Information for Your State

When you register for the Tour de Cure you become a part of the American Diabetes Association’s largest fundraising event. Joining you will be thousands of participants from around the country who come together to directly impact the lives of people facing the daily challenges of diabetes and their loved ones. Fundraising is easy with your personal fundraising center. By volunteering for Tour de Cure, you will be doing a great part to improve the lives of those affected by diabetes. Individuals and families with incomes below a certain level who do not have an offer of affordable insurance from their employer and do not qualify for certain other types of coverage may be eligible for financial help to purchase insurance. Participating in the Tour de Cure is an experience you will never forget. The event is very well organized and has routes available for all skill levels.

Located at the famous Topsfield Fairgrounds, you will leave on one of four fantastic rides, then cross the finish line and enjoy a festival full of fun, food, family, and friends! Help us make an impact on those living with diabetes. When you fill out an application in the Marketplace, you will be informed about your eligibility for financial help to purchase insurance, or if you’re eligible for coverage under your state’s Medicaid or CHIP. The verification code for Visa, Master Card, and Discover is a 3-digit number printed on the back of your card. It appears after the account number or last 4-digits of your account number, in the signature panel. A true test of endurance for the experienced cyclist, this route includes two timed segments for fantastic prizes, challenging climbs and a variety of terrain in Massachusetts and Southern New Hampshire. Volunteering with friends and family makes the day even more memorable.

It is up to each state to set its Medicaid program eligibility standards for adults and children. Contact your state Medicaid program or the state Health Insurance Marketplace for more information on who is eligible for Medicaid in your state. The children’s health insurance program (CHIP) is designed to provide coverage to children and teens up to age 19 whose families may have too great an income or assets to qualify for Medicaid, but who may not be able to afford health insurance. Contact the CHIP program or the state Health Insurance Marketplace for more information. Once you register for a specific task, you’ll be added to that group communication list. Contact Medicare for more information on who is eligible and what is covered. Also, free health insurance counseling and personalized assistance for Medicare beneficiaries is available in every state through the State Health Insurance Assistance Program (SHIP).

Call 1-800-MEDICARE (1-800-633-4227) or visit www.shiptacenter.org to find the telephone number of the SHIP office closest to you. If you or your loved one is unable to see a physician due to the cost of care, there may be a local community health clinic in your area. These clinics generally are free to patients or require a very small fee.
