Diabetes and the Hemoglobin A1C Test

For the main analyses, among patients who are adherent to their antidiabetic medications, multivariable logistic regression analysis was used to examine patient characteristics and type of medication related to achievement of target A1c levels. The Diabetes Autoimmunity Study in the Young (DAISY) has followed for development of islet autoimmunity and diabetes general population newborns carrying human leukocyte antigen (HLA) genotypes conferring risk for type 1 diabetes and young siblings or offspring of people with type 1 diabetes. But now, several recent A1C studies have shown a J-shaped relationships, in which at the lower end some bad things happen, at the center things are better, and at the top end things are terrible. A1C levels were decreased by thyroid hormone replacement. The percentage of hemoglobin in a blood sample that is glycosylated (has glucose attached to it) indicates how well blood glucose has been controlled over the previous two to three months. In patients 75 years or older, infection rates were highest when HbA1c levels were higher than 9.0%. There is no generally used simple formula for converting these types of values into an estimated average glucose level throughout the day.

In this study, we evaluated whether the use of an IBGMS would improve the outcome of treatment for patients with type 2 diabetes. Exercising after eating is particularly recommended to ensure you have adequate energy for your exercise (such as a brisk walk) and can keep the blood sugar from spiking after eating. A1c is frequently used to help newly diagnosed diabetics determine how elevated their uncontrolled blood glucose levels have been over the last 2-3 months. The higher the hemoglobin A1c percentage, the greater the risk of developing diabetic eye, kidney, cardiovascular, and nervous system disease. No preparation is necessary. One of the advantages of this test is that you do not have to fast before you take it. The ADA doesn’t say here what they consider a normal A1C level to be.

In addition, thyroid hormone promotes albumin metabolism, and albumin degradation is reduced in hypothyroidism (9). There are also portable machines that can determine the value from a fingerstick – these can be performed in physician offices or even in a patient’s home. The hemoglobin A1c percentage rises as your average blood sugar level rises. These results will help patients and providers set realistic day-to-day SMBG targets to achieve individualized HbA1c goals. A1C values were measured using the ADVIA Centaur Immunoassay System (Tarrytown, NY). For instance, an A1c level of 7% correlates to an average hourly blood glucose of 170, whereas an A1c level of 9% correlates to an average hourly blood glucose of 240. For example, a person with heart disease who has lived with type 2 diabetes for many years without diabetic complications may have a higher A1c target (e.g., 7.5%-8.0%) set by their healthcare provider, while someone who is otherwise healthy and just diagnosed may have a lower target (e.g., 6.0%-6.5%) as long as low blood sugar is not a significant risk.

If you have been diagnosed as diabetic and your test is not normal, your health care provider will talk to you about how to lower your blood sugar through diet (weight loss and eating the proper proportions of carbohydrates, protein, and fat), exercise, and/or medicine. Keeping your blood sugar levels and hemoglobin A1c levels in or near normal ranges will help you avoid the complications of diabetes, such as eye disease, kidney disease, or nerve damage. The A1C test should be done every 3 months unless you are in good control, e.g., less than 7%. This “consensus group” of experts defined cure as complete remission for a long enough time that the risk of recurrence is very low.
